Intake Process

Complete the initial assessment
The intake process begins with an initial assessment with a counselor/therapist to gather information about you, as well as explore your strengths and challenges and identify your desired goals.

Create a treatment plan.

​Based on the comprehensive initial assessment, the counselor/therapist will partner with you to create a treatment plan. The treatment plan will contain specific goals and interventions that you and the counselor/therapist have agreed on. Your treatment plan may focus on the following topics:

Your concern(s) for seeking out treatment

Your history and the progression of your concern(s)

Your strengths and resources
-
Your concerns for safety (e.g., abuse, current risk of self-harm, previous suicide attempts)
-
Your physical and behavioral health issues
-
Your social supports and environmental influences (e.g. work situation, income, living situation, neighborhood, family background, friends etc.)

After completion of the assessment and treatment plan, the counselor/therapist will recommend a course of treatment that best fits your current concerns and needs. These treatment options may consist of the following:

individual therapy

group therapy
-
family support
-
peer support
-
psychiatric services
​
You and your counselor/therapist will discuss the frequency of sessions, number of sessions, goals, and type of counseling (individual and/or group). These will also be identified on your treatment plan. You and your counselor/therapist will re-evaluate the frequency of your sessions as situations arise and as you progress through your treatment.

We provide integrated outpatient mental health and substance use services, as well as continuing care for ongoing recovery support.
We provide treatment for a variety of conditions, such as bereavement, partner and parenting problems, domestic violence, treatment of mental illnesses, trauma, and anger management.
